What it means

For example: A minor procedure is billed with its dressing and its consultation as three separate lines.

The rejection is about the claim as it was sent, not about whether the care was right.

Why it usually happens

  • Parts of one procedure were billed line by line.
  • The contract bundles these services into one price.

How to fix this claim

  1. Replace the separate lines with the bundled code from the contract.
  2. Resend before the deadline.
Mind the deadline. You have 15 days from the rejection to answer it (CHI Unified Health Insurance Contract, clause 5.4). Your contract with the insurer may set a different window, so check it.

How to stop it next time

Load each insurer’s contract price list, including its bundles.

This page explains the common causes. Your insurer’s policy and your contract decide each claim.

Before sending

Nabdah catches this before you send.

Claims are priced from each contract’s price list, so bundles are billed the way the contract sets them.
