There is a diagnosis on the claim, but the insurer doesn’t accept it as a reason for this service.

What it means

For example: A vitamin D test is claimed with a diagnosis of a common cold.

The rejection is about the claim as it was sent, not about whether the care was right.

Why it usually happens

  • The diagnosis and the service don’t match under the insurer’s rules.
  • A more specific diagnosis was needed.
  • The wrong diagnosis was linked to the service line.

How to fix this claim

  1. Check which diagnosis justifies the service, from the visit note.
  2. Correct the code or the link, and resend.
  3. If the diagnosis is right, appeal with the clinical notes before the deadline.
Mind the deadline. You have 15 days from the rejection to answer it (CHI Unified Health Insurance Contract, clause 5.4). Your contract with the insurer may set a different window, so check it.

How to stop it next time

Use the most specific diagnosis the visit notes support.

This page explains the common causes. Your insurer’s policy and your contract decide each claim.

After the rejection

Nabdah keeps this rejection in front of you.

Diagnoses come from the signed visit note and are linked to each line, so the reason travels with the claim. Each rejection sits in today’s list with its reason and its appeal date, so your biller works it in time.
